J-1 exchange visitors may request DS-2019 documents to bring their dependent family
members to the U.S. after their own arrival at UCSB. To apply for the form DS-2019
for dependents (defined as spouse and unmarried children under the age of 21), log
on to the UCSB SEVIS page at:
- Enter the information for each dependent in the “Dependent” tab.
- You will need
to update your financial information to show additional support for each dependent
on your J-1 program.
A DS-2019 form will be issued to each dependent for which a
request was made. The dependents will apply for the J-2 visa at the U.S. consulate
abroad. In addition to their own DS-2019 form, they will need the following documents

- Supporting
letter from the Sponsoring Department at UCSB.
- Evidence of financial support.
(refer to financial minimum chart).
- Proof of relationship to the J-1 exchange
visitor.
Once your family member(s) arrive in Santa Barbara, please bring their
passports, DS-2019 forms, I-94 cards, and proof of health insurance to OISS for
registration.
